Join us at Hospice House for a two-part series focused on Advance Care Planning (ACP)—an essential process for thinking ahead about your health and personal care.
What is Advance Care Planning?
Advance Care Planning is the process of reflecting on your values, beliefs, and wishes for future health and personal care. It’s about having conversations with the people you trust and ensuring that if you're ever unable to make decisions for yourself, your care reflects your wishes.
At this session, you'll learn:
How to start the conversation about your care preferences
The importance of choosing a substitute decision-maker
How ACP can help ensure you receive the care that’s right for you, even if you can't speak for yourself
Advance Care Planning is a vital part of life planning, just like preparing a will or saving for retirement. Planning ahead for your future health and care is one of the most empowering steps you can take for yourself and your loved ones.
